Pentair Hyper Flow, PacFab Hyper flow, American Products 4 7/8" (125mm) square spa suction cover.

Common on Monarch, Maax, and many other spas made in the 1990s & 2000s.

These are no longer available, or legal. VGB-compliance has banned these suctions. The suctions must be upgraded. 

The CMP suction here is a popular suction. Note that it will need to be replumbed to suit.
