Notes:

Do not touch lamp with bare skin or allow any oils or other contaminents onto glass. If this happens use non-residue isopropyl alcohol to clean lamps. Handle with extreme care.
UV Lamps have small metallic beads that rattle around in the lamp. This is normal. These are vaporised when high-voltage is applied, which in turn creates the UV light.
When plugging in the lamp, ensure the plug is carefully aligned with the pins and gently pushed into place. Forcing the plug or misaligning it can damage the lamp's pins, which is not covered under warranty.
